VFS is the only group staffed exclusively by physicians specializing in aerospace medicine that can provide confidential information to all pilots and controllers. Our physicians specialize in obtaining rapid dispositions and informed advocacy with the FAA Aeromedical Certification Division. Don't risk your medical certificate with uncertainties about reporting medical conditions -- consult a VFS physician.
Select the appropriate Confidential Questionnaire from the left navigation. Get accurate, fast, complete answers to your questions through secure e-mail. Once submitted, a VFS physician will respond within 2 business days.
Select the Individual Inquiry Confidential Questionnaire to submit a question to one of the VFS physicians. This confidential service is offered on a fee-for-use basis. The fee for each questionnaire is $39.95. Telephone access is also available at 1-866-AEROMED (1-866- 237-6633) for a service fee of $50.
Select the NATCA Member Confidential Questionnaire if you are a NATCA member in good standing and wish to submit a question to a VFS physician. The service is provided to you at no cost as a NATCA member benefit.
Select the Corporate & Partnership Confidential Questionnaire if you receive benefits as a member of a union, an aviation organization or through your corporate flight department. This service may be provided to you at a reduced fee or no cost, depending on your affiliation.
